Community Health Workers

Community Health Workers assist individuals with a wide range of service needs. Their support can range from providing referral options or helping clients determine their eligibility for services to more hands-on assistance, such as making phone calls, completing forms, or following up on application processes. They also help clients manage and navigate their Mi Bridges accounts to ensure access to necessary resources.

If someone has a need and doesn’t know where to start, our CHWs can help point them in the right direction!
