Community Health Workers

Community Health Workers help people with any service need they have. From the basic just giving them some referral options or helping someone determine if they are eligible for a service, all the way up to making phone calls with the client, helping them complete forms or follow up on an application process, to helping a person manage their Mi Bridges account and how to access it. 

If someone has a need and doesn’t know where to start, our CHWs can help point them in the right direction!
